A runner comes in on a Monday with a pair of road shoes and a grazed knee. Nothing dramatic: they had followed a friend off the pavement onto a wet grass bank, put a foot down at an angle, and the shoe simply slid. They wanted to know whether the shoes were faulty. They were not. They were doing exactly what a smooth outsole does on wet grass, which is nothing at all.
Road and trail shoes look similar enough on a shelf that the difference gets underestimated. Underneath, they are built around opposite priorities: one is trying to be efficient on a hard, predictable surface, the other is trying to keep you upright on a soft, unpredictable one.

Grip is the headline, but protection is the quiet one
Lugs are what everyone notices, and they are the main event on soft ground: rubber teeth that push into mud and grass rather than skating over it, with gaps wide enough that the mud falls out again instead of packing into a smooth pad.
The less obvious difference is stiffness. A trail shoe resists twisting and folding, which matters when you land on the edge of a rock or a tree root. Many include a rock plate, a thin sheet of firmer material sandwiched under the foam, so a stone that would leave you hopping in a road shoe registers as a dull nudge instead. Anyone who has run a stony bridleway in road shoes for an hour knows precisely what that feels like without it.
Uppers earn their keep in ways you only notice later
Road uppers are built to be light and to breathe. Trail uppers are built to survive being scraped along gorse, dragged through brambles and repeatedly soaked. The reinforcement around the toe and the sides is there because off-road terrain attacks the shoe from angles tarmac never does, and a thin road mesh tears where a trail upper shrugs.
What happens when you use the wrong one

They work, in the sense that you will complete the run. But those lugs are standing you off the ground on small isolated points of rubber, which feels vague and slightly unstable on a hard flat surface, particularly around corners. Worse, tarmac grinds lugs down fast. A few weeks of road commuting can round off an outsole that would have lasted a year in the hills, and once the lugs are blunt the shoe is no longer much good off-road either. It is an expensive way to make a shoe mediocre at both jobs.
Road shoes on wet grass and mud
Here the failure is immediate rather than gradual. A smooth outsole on wet grass has essentially no grip, and the moment there is a slope or a change of direction the foot goes. Add the lack of underfoot protection on stones and the softer heel hold on uneven ground, and you have the runner with the grazed knee. For a short section of firm towpath, fine. For a proper off-road route in a British winter, no.
Most UK running is mixed, and that is the useful bit
The road vs trail running shoes question is usually posed as a choice, and for most people it is not one. Very few of us drive to a mountain. A typical route round here is a mile of pavement, a stretch of park, a muddy path along a river, then pavement home. Neither specialist shoe is built for that combination, which is why the door-to-trail category exists and why it is the right answer for more runners than either extreme.
Door-to-trail shoes use shallower, more closely spaced lugs, enough to hold on damp grass and packed dirt without feeling like stilts on tarmac. They are lighter than full trail shoes, keep some underfoot protection and usually have a slightly tougher upper than a road model. They are a compromise, openly, and compromises are the correct choice when the terrain is a compromise too.
Go for a full trail shoe when the ground is genuinely soft, steep or rocky for most of the run, when you are heading out in winter mud, or when you have entered an off-road race. Stay with a road shoe when nearly everything you do is on pavement, because the lighter, smoother option is simply better at that job. If you end up with two pairs, they will both last longer, because each is doing the work it was designed for and neither is being destroyed by the wrong surface.
